A Quality Assurance Team member will email or call you within 24 hours to acknowledge receipt of your e-mail. You will be contacted within 3 days to discuss your request, and schedule an appointment.
Please do not submit emergency requests on a service request form. All emergency requests must be communicated by phone to ensure the most accurate and efficient exchange of information.
